If you would like to be referred for an ADHD assessment, please complete an ASRS v1.1 screening questionnaire and provide a written statement outlining how your potential ADHD symptoms have affected different areas of your life so far.
These can be emailed to sherbourne.pm@nhs.net or dropped in at reception with details of who you'd like to be referred to.
Options for referral are the Adult Neurodevelopmental Team in Coventry or a Right to Choose provider. To help you decide which option is best for you, further information about these services is available on the ADHD UK website.
Please note if you're considering medication and would like us to potentially prescribe this, the provider needs to have a Dr within their team, and they must initiate and stabilise medication, and keep you on for follow up.
